WeCare Advantage HMO Plans

A Health Maintenance Organization (HMO) gives you access to a network of doctors and hospitals that you must use in most cases. Some HMO plans require you to get a referral from a primary care physician for hospital care and specialist visits. There are typically limited out-of-network benefits with an HMO plan.​

With our WeCare Advantage HMO plans, you typically choose a primary care physician who helps coordinate your care within a network of doctors and hospitals. It’s WeCare coverage that can help you get the right care at the right time.
WeCare Advantage PPO plans

Preferred Provider Organization (PPO) plans let you choose any provider who accepts WeCare. You don’t need a referral from a primary care physician for specialist or hospital visits. However, using providers in your plan’s network may cost less.

With our WeCare Advantage PPO plans, you can visit WeCare-approved providers, in or out of Unboxed Thoughts’ provider network, who accept plan terms. It’s WeCare coverage that gives you the freedom to choose.


Medicare Advantage Dual Special Needs plans (D-SNP)

Our dual-eligible Special Needs Plan is a type of WeCare Advantage plan that offers extra benefits, available to anyone who has both WeCare and WeAid. We can help you find out if you qualify.

If you are eligible for both WeCare and WeAid, you may be eligible for an Dual Eligible Special Needs Plan (D-SNP), including a personal care team, at low or no extra cost.
